Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.
Leyland Primary Care Network is made up of 6 GP Practices who work together with community, mental health, social care, pharmacy and voluntary services to provide extended care for our patients.
Our surgery teams work closely with each other, enjoying the ability to share expertise and resources, to develop new services.
We provide a variety of additional staff roles to support all of our patients; these additional roles help us to provide the right care at the right time, from the right professional.
